Contractual Guarantee Form for Patients in an Auto Accident
Patients receiving treatment as a result of an automobile accident with a third party involved will be asked to sign this Contractual Guarantee of Payment for Health Care Services. We will require this contract if your treatment continues after your PIP (auto) and health insurance benefits have been exhausted. You will need to have your attorney sign this contract as well. The payment arrangements you select in this form will take effect after all insurance benefits have been exhausted. You can fill out this form anytime prior to exhausting your benefits or receiving Out of Benefits letters from either your auto or health insurance company.
